ROUTINE MEDICATION REVIEWS
Routine medication reviews are undertaken by a doctor or pharmacist annually. You may notice you are unable to order your repeat medication if a medication review if due. In this instance, repeat medication can be ordered by typing the medication you require into the notes section.
Most medication reviews are undertaken automatically by a doctor/ pharmacist with no need to contact you.
If you require a blood test, blood pressure check or an appointment with one of our pharmacy team our reception team will contact you.
LONG TERM CONDITION REVIEWS
Long term conditions include diabetes, asthma, COPD and heart failure. If you have a long term condition you will be contacted for a review at least annually.

- ASTHMA – If you are due a review please us the following link (SystmOnline) to complete an ACT asthma questionnaire
- COPD – if you are due a review please call our admin team on: 01920 483227
- HEART FAILURE – if you are due a review please call our admin team on: 01920 483227
- Our patients with mental health conditions and learning disabilities are also entitled to an annual review. You will be contacted regarding this however if you think you have missed a review please call our secretarial team on 01920 468777 & select option 2
